Mission: To improve the health and wellness of individuals living with or at risk of diabetes through equitable, accessible, compassionate, high-quality care.

The Diabetes Foundation (DF) is a comprehensive and reliable source of help, hope and better health for children, adults and their families living in New Jersey.

Incorporated in 1990, the Diabetes Foundation exists to help children and adults living in distress caused by this overwhelming chronic disease. Over the past 30 years, DF has expanded its offerings, evolving into a comprehensive “one-stop shop” for individuals with any form of diabetes, providing free diabetes supplies, transportation, A1C screenings and diabetes camp, etc. Since its founding, the Diabetes Foundation has assisted over 10,000 people and their families, helping them manage their diabetes, control their blood sugar levels, and lead healthier lives.

The Diabetes Foundation offers free education, social support and financial assistance in partnership with medical, health and wellness professionals to ensure that individuals with prediabetes, Type 1, Type 2, or gestational diabetes have the knowledge and tools necessary to reduce their risk of developing diabetes or to delay or avoid the long-term complications of living with uncontrolled blood sugar.
